Family Contact Information Request Form

The Prairie School evaluates every request for family contact information on a case-by-case basis, and reserves the right to follow up or deny any request as deemed necessary.

• All requests must be made at least two weeks in advance of the date information is needed (specified as "Due Date" in the form below). 

• Please note that out of respect for their privacy, families have had the opportunity to opt out of having their contact information shared with the wider TPS community. Any contact list shared will reflect those wishes.
